What type of procedure you will be having (usually provided by your surgeon’s office),
The name of the doctor who will be performing the procedure and at what hospital/facility.
The estimated duration of your procedure (usually provided by your surgeon’s office).
Any additional procedures that will be performed.
Whether or not you are insured.
